Overview
What Is Male Infertility?
Male infertility refers to difficulty achieving pregnancy due to problems affecting sperm production, sperm quality, sperm delivery or reproductive function. Evaluation commonly includes medical history, physical examination, semen analysis and further tests when required.
Two important causes are Varicocele and Azoospermia. Varicocele is an enlargement of veins around the testicle that may affect sperm production and semen quality. Azoospermia means that no sperm are detected in the ejaculate and may occur because of a blockage or reduced sperm production in the testes.
Important: Male infertility does not have a single treatment for every patient. The correct approach depends on the underlying cause and may include medical treatment, varicocele surgery, correction of reproductive tract obstruction, sperm retrieval procedures or assisted reproductive techniques.
Causes
What Causes Male Infertility, Varicocele & Azoospermia?
Varicocele
Enlarged veins around the testicle can raise testicular temperature and may affect sperm production and quality.
Hormonal Imbalance
Abnormal testosterone or other reproductive hormones can interfere with normal sperm production and fertility.
Genetic Causes
Chromosomal abnormalities or Y-chromosome microdeletions can cause very low sperm counts or azoospermia.
Reproductive Tract Blockage
Blockage in the vas deferens or ejaculatory ducts can prevent sperm from reaching the semen, causing obstructive azoospermia.
Testicular Problems
Previous infection, injury, surgery or impaired testicular function may reduce or completely stop sperm production.
Lifestyle & Medical Factors
Smoking, obesity, certain medications, medical treatments and environmental factors may negatively affect male fertility.
Symptoms

- Hormonal or reproductive abnormalities
Diagnosis
How We Diagnose Male Infertility, Varicocele & Azoospermia
Accurate diagnosis helps identify whether infertility is related to sperm production, varicocele, hormonal imbalance, blockage or another underlying condition. Evaluation is customised according to the patient’s history and test findings.
- Semen Analysis – checks sperm count, movement, shape and semen volume.
- Physical Examination – evaluates the testes, reproductive organs and checks for a clinical varicocele.
- Hormonal Tests – testosterone, FSH and other hormones may be assessed when indicated.
- Scrotal Ultrasound – may help evaluate testicular abnormalities or varicocele in selected cases.
- Genetic Testing – may be advised in men with azoospermia or severely reduced sperm counts.
- Specialised Testing – additional investigations may be required to differentiate obstructive from non-obstructive azoospermia.

Frequently Asked Questions
Male infertility occurs when problems with sperm production, sperm quality, sperm delivery or reproductive function reduce the chance of achieving pregnancy.
A varicocele is an enlargement of veins within the scrotum. In some men, a clinical varicocele can negatively affect sperm production and semen quality.
Azoospermia means that no sperm are detected in the ejaculated semen. It may occur because sperm production is impaired or because a blockage prevents sperm from reaching the semen.
Yes. Varicocele can be associated with abnormal semen parameters and reduced fertility in some men, although not every man with a varicocele is infertile.
Azoospermia is broadly classified as obstructive azoospermia, where sperm transport is blocked, and non-obstructive azoospermia, where sperm production within the testes is significantly impaired.
Evaluation may include medical and reproductive history, physical examination and semen analysis. Hormonal tests, genetic testing or imaging may also be recommended depending on the findings.
Treatment depends on its cause. Options may include treatment of hormonal problems, microsurgical correction of certain blockages, surgical sperm retrieval or assisted reproductive techniques such as IVF/ICSI.
In appropriately selected men with a palpable varicocele, infertility and abnormal semen parameters, varicocele repair may improve semen parameters and pregnancy chances. The benefit varies between patients.
In some cases, yes. Depending on the underlying cause, sperm may be retrieved surgically and used with assisted reproductive techniques. For selected men with non-obstructive azoospermia, Micro-TESE may be considered for sperm retrieval.
A specialist evaluation should be considered when a couple has difficulty conceiving, when semen analysis is abnormal, or when conditions such as varicocele or azoospermia are identified. Proper evaluation helps determine the underlying cause and appropriate treatment options.
