What is Testicular Pain (Orchialgia)?
Testicular pain, also known as orchialgia when persistent or recurrent, refers to pain or discomfort felt in one or both testicles or surrounding scrotal structures.
Pain may be sudden and severe or develop gradually over time. It can result from infection, injury, varicocele, previous surgery, nerve-related pain or other scrotal conditions. In some chronic cases, no clear underlying cause is identified.
Important: Sudden, severe testicular pain is a medical emergency because it may indicate testicular torsion, where the blood supply to the testicle becomes twisted. Immediate assessment is essential because treatment delay can result in permanent testicular damage.
Causes
What Causes Testicular Pain?
Infection
Epididymitis or orchitis can cause pain, tenderness and swelling in the scrotum.
Testicular Torsion
Twisting of the spermatic cord can suddenly interrupt blood flow and cause severe testicular pain.
Varicocele
Enlarged veins around the testicle may cause a dull, aching or heavy sensation in some patients.
Injury or Trauma
Sports injuries, accidents or direct trauma to the scrotum can result in pain and swelling.
Previous Surgery
Pain may occur after procedures such as vasectomy, inguinal hernia surgery or other operations involving the groin or scrotum.
Nerve-Related Pain

Diagnosis
How We Diagnose Testicular Pain
A detailed evaluation is required to identify treatable causes such as infection, torsion, trauma, testicular masses or varicocele before diagnosing chronic scrotal pain syndrome.
- Medical History – The doctor evaluates when the pain started, its severity, previous surgery, injuries, infections and associated urinary or sexual symptoms.
- Physical Examination – The testicles, epididymis, spermatic cord, groin and surrounding structures are examined for tenderness, swelling or abnormalities.
- Urine & Laboratory Tests – Tests may be recommended when infection or another underlying medical condition is suspected.
- Scrotal Doppler Ultrasound – Ultrasound may be used when clinically indicated to evaluate acute scrotal conditions, blood flow or structural abnormalities.
- Spermatic Cord Block – In selected chronic cases, a spermatic cord block may help determine whether nerve pathways within the cord are contributing to the pain and may help guide further treatment.
