Overview
What is Circumcision?
Circumcision is a surgical procedure in which part or all of the foreskin covering the head of the penis is removed. It may be performed for medical, hygiene, cultural or personal reasons.
In adults, circumcision is commonly considered for conditions such as phimosis, recurrent balanitis, painful foreskin retraction, repeated foreskin infections or scarring of the foreskin.
The procedure allows the glans to remain exposed and can help prevent repeated problems caused by a tight or diseased foreskin.
Important: Not every foreskin problem requires circumcision. Treatment depends on the underlying condition, severity of symptoms, previous treatments and individual preference.
Indications
Who May Need Circumcision?
Phimosis
A condition in which the foreskin is too tight to retract comfortably over the head of the penis.
Recurrent Balanitis
Repeated inflammation or infection involving the head of the penis and foreskin.
Painful Foreskin Retraction
Pain, tearing or discomfort while retracting the foreskin during hygiene or sexual activity.
Recurrent Foreskin Infections
Repeated fungal or bacterial infections may lead to inflammation, scarring and persistent discomfort.
Balanitis Xerotica Obliterans / Lichen Sclerosus
A chronic inflammatory condition that may cause whitening, thickening, narrowing or scarring of the foreskin.
Paraphimosis
A tight foreskin becomes trapped behind the head of the penis and cannot be brought forward again. Acute paraphimosis requires urgent medical treatment.

Evaluation
How We Evaluate Before Circumcision
Before recommending circumcision, Dr. Gautam Banga evaluates the foreskin, penile anatomy and underlying cause of the symptoms.
Evaluation may include:
- Medical History – duration of symptoms, previous infections and previous treatment.
- Physical Examination – assessment of foreskin tightness, scarring, inflammation and penile anatomy.
- Infection Assessment – active balanitis or local infection may require treatment before surgery.
- Diabetes Screening – recurrent genital infections can sometimes be associated with uncontrolled blood sugar.
- Assessment for Lichen Sclerosus – scarring or whitening may indicate an underlying inflammatory condition.
- Urinary Symptoms Review – especially if foreskin narrowing affects urination.
- Treatment Counselling – discussion of procedure options, recovery, hygiene and possible complications.

Frequently Asked Questions
Circumcision is a surgical procedure in which the foreskin covering the head of the penis is partially or completely removed.
Common medical reasons include phimosis, recurrent balanitis, painful foreskin retraction, repeated infections, lichen sclerosus and recurrent paraphimosis.
Phimosis is a condition in which the foreskin cannot be comfortably pulled back over the head of the penis.
It can be caused by tightness, inflammation, infection or scarring.
No.
Some mild cases may improve with appropriate topical treatment and gentle stretching. Circumcision is more likely to be considered when the foreskin is heavily scarred, symptoms are recurrent or conservative treatment has failed.
The procedure is performed under anaesthesia, so surgical pain is controlled during the operation.
Temporary soreness, swelling and sensitivity are expected during recovery and can usually be managed with prescribed medication.
The duration depends on the surgical technique and complexity of the foreskin condition, but circumcision is generally a relatively short procedure.
Most patients gradually return to routine activities within a short period, but complete wound healing takes longer.
Heavy exercise and sexual activity should be avoided until the surgeon confirms adequate healing.
Sexual intercourse and masturbation should be avoided until the surgical wound has healed sufficiently.
The exact timing varies, but patients should follow their surgeon's advice rather than resuming activity based only on pain improvement.
Temporary increased sensitivity of the glans is common after surgery because it was previously covered by the foreskin.
Long-term sexual experience varies between individuals. Circumcision is performed to treat a medical problem and should not be promoted as a guaranteed way to increase sexual pleasure or performance.
Circumcision can reduce repeated foreskin-related inflammation in appropriately selected patients, particularly when recurrent balanitis is associated with phimosis.
Any active infection should still be appropriately diagnosed and treated.
