What Is Chronic Pelvic & Perineal Pain?

Chronic pelvic and perineal pain refers to persistent or recurrent pain in the lower pelvis, perineum, genital region, bladder area or surrounding pelvic floor muscles. The pain may feel like burning, pressure, heaviness, aching or sharp discomfort and can sometimes worsen during sitting, urination, bowel movements or sexual activity.

The condition may be associated with pelvic floor muscle dysfunction, chronic prostatitis/chronic pelvic pain syndrome, bladder pain, nerve irritation, previous surgery, trauma or infection. In some patients, no single structural cause can be identified.

Chronic pain may also involve changes in how the nervous system processes pain, so treatment often requires more than one approach.

Important: Effective management begins with identifying treatable causes and understanding whether the pain is primarily muscular, neurological, urinary, inflammatory or multifactorial.

How We Evaluate Chronic Pelvic & Perineal Pain

A detailed evaluation is important because chronic pelvic pain can involve urological, muscular, neurological and other pelvic structures.

  • Medical History – reviews duration, location and pattern of pain.
  • Urinary & Sexual History – assesses associated bladder and sexual symptoms.
  • Physical Examination – evaluates the abdomen, genital and perineal region.
  • Pelvic Floor Assessment – checks for muscle tightness, tenderness and trigger points.
  • Urine Tests – help identify urinary infection or other abnormalities.
