Overview
What Is a Urethral Diverticulum?
A urethral diverticulum is an abnormal pouch or sac that forms along the urethra, the tube that carries urine out of the body. Urine can collect inside this pouch, leading to infection, discomfort, dribbling or difficulty with urination.
Urethral diverticula are more common in women but can also occur in men, particularly after urethral stricture, trauma, infection or previous urethral surgery. Trapped urine may contribute to recurrent urinary tract infections, post-void dribbling, pain or swelling around the urethra.
In some cases, the diverticulum may gradually enlarge or develop stones because of persistent urine stagnation. Symptoms and severity depend on its size, location and connection with the urethra.
Important: Symptoms can resemble other urinary conditions. Proper evaluation is necessary to identify the size, location and any associated urethral obstruction before treatment.
Causes
Causes of Urethral Diverticulum
Urethral Infection
Repeated infection may weaken urethral tissues.
Urethral Stricture
Obstruction may increase pressure inside the urethra.
Previous Surgery
Urethral procedures may occasionally lead to diverticulum formation.
Trauma
Injury can damage the urethral wall.
Blocked Glands
In women, infected periurethral glands may contribute to diverticulum formation.
Congenital Causes
Rarely, a diverticulum may be present from birth.
Symptoms
Signs & Symptoms of Urethral Diverticulum
- Post-void urine dribbling
- Recurrent urinary infections
- Pain or burning while urinating
- Weak or interrupted urine flow
- Feeling of incomplete emptying
- Pelvic or urethral discomfort
- Urine leakage
- Swelling or discharge near the urethra
Diagnosis
How We Diagnose Urethral Diverticulum
Accurate diagnosis helps determine the location, size, complexity and associated urethral problems.
- Medical History – reviews urinary symptoms and previous procedures.
- Physical Examination – checks for swelling or tenderness around the urethra.
- Urine Tests – identify infection or blood in the urine.
- MRI – particularly useful for detailed evaluation of female urethral diverticula.
- Ultrasound – may help identify a fluid-filled pouch.
- Cystoscopy – allows direct examination of the urethra.
- VCUG / RGU – may demonstrate the diverticulum and associated narrowing.
- Uroflowmetry – evaluates urinary flow when obstruction is suspected.

Frequently Asked Questions
A urethral diverticulum is an abnormal pouch or sac that forms alongside the urethra. Urine can collect inside this pouch, which may lead to dribbling, discomfort, recurrent infections or difficulty emptying the bladder completely.
It may develop due to recurrent infection, urethral obstruction, trauma, previous urethral surgery or congenital abnormalities. In men, urethral stricture and previous procedures are common associated factors.
Yes. Although urethral diverticula are more commonly discussed in women, men can also develop them. In men, they may occur after urethral stricture, trauma, infection, catheterisation or previous urethral reconstruction.
Common symptoms include post-void dribbling, weak urine flow, recurrent urinary infections, pain while urinating and incomplete bladder emptying. Some patients may also notice swelling, discharge or urine leakage near the urethra.
Yes. Urine can remain trapped inside the diverticulum after urination, creating an environment where bacteria can grow. This may lead to repeated urinary tract infections, burning urination or persistent urinary discomfort.
Diagnosis usually begins with a detailed medical history and physical examination. Depending on the case, tests such as MRI, ultrasound, cystoscopy, VCUG or retrograde urethrogram may be used to identify the size, location and connection of the diverticulum.
No. Small diverticula that do not cause significant symptoms or complications may sometimes be monitored. Surgery is more commonly considered when there are recurrent infections, stones, urinary obstruction, persistent dribbling or significant discomfort.
Diverticulectomy is a surgical procedure in which the abnormal urethral pouch is removed. The urethral wall is then carefully repaired or reconstructed to restore normal urine flow and reduce the risk of further complications.
Recurrence is possible, especially when there is significant scar tissue, infection, previous urethral surgery or an untreated urethral stricture. Proper evaluation and treatment of the underlying cause can help reduce the risk of recurrence.
You should seek evaluation if you have persistent post-void dribbling, recurrent UTIs, weak urine flow, urethral swelling, pain or difficulty emptying your bladder. Early diagnosis can help identify associated obstruction or infection before complications become more severe.
