What Is Phimosis?
Phimosis is a condition in which the foreskin is too tight to be pulled back fully over the head of the penis (glans). In adults, it may cause pain, tightness, difficulty with hygiene, discomfort during erections or sexual activity, and sometimes urinary problems.
Phimosis may develop due to scarring, repeated infection, inflammation or skin conditions such as lichen sclerosus (BXO). A proper examination is important to identify the underlying cause and decide whether treatment should be medical or surgical.
Important: The foreskin should never be forcefully retracted. Repeated cracking or tearing can worsen scarring and make phimosis more severe. Early evaluation helps prevent recurrent infection, discomfort and long-term foreskin damage.

What Causes Phimosis?
Lichen Sclerosus (BXO)
In adults, pathological phimosis is commonly associated with lichen sclerosus/BXO, which can cause inflammation, whitening and scarring of the foreskin.
Recurrent Balanitis
Repeated inflammation or infection of the glans and foreskin can contribute to irritation and progressive foreskin tightness.
Infection & Inflammation
Bacterial, fungal and inflammatory conditions affecting the foreskin may lead to swelling, discomfort and recurrent foreskin problems.
Repeated Tearing
Forceful retraction or repeated cracking of a diseased foreskin can worsen scarring and make phimosis more severe.
Diabetes

- Urine Test – can check for infection and glucose in the urine.
- Swab Testing – may be used when bacterial infection or discharge is suspected.

Phimosis Treatment Options at Urethra & Penile Surgery
Foreskin-Preserving
Preputioplasty may have a limited role in selected patients who wish to preserve the foreskin, while frenuloplasty may be appropriate when symptoms are mainly caused by a short frenulum rather than true foreskin narrowing. Preputioplasty has a limited role in adults with active lichen sclerosus.
