What Is Meatal Stenosis?
Meatal stenosis is a condition in which the urinary opening (urethral meatus) at the tip of the penis becomes abnormally narrow. This narrowing can restrict the normal flow of urine and may cause a weak, thin, spraying or deflected urinary stream.
Patients may also experience difficulty passing urine, straining, burning during urination or incomplete bladder emptying. In some cases, narrowing may be associated with previous urethral surgery, trauma, instrumentation or inflammatory conditions such as lichen sclerosus.
Important: Meatal stenosis may sometimes be part of a longer distal urethral stricture. Proper evaluation is important to determine the exact extent of narrowing before selecting treatment. Current EAU guidance recommends open meatoplasty or distal urethroplasty for appropriately selected patients with meatal or distal urethral strictures.

What Causes Meatal Stenosis?
Previous Urethral Surgery
Meatal narrowing can occur after procedures involving the urethra, including previous hypospadias repair or other reconstructive surgery.
Lichen Sclerosus (BXO)
Lichen sclerosus is an inflammatory scarring condition that can affect the penile skin and distal urethra and contribute to meatal stenosis.
Urethral Instrumentation
Repeated cystoscopy, catheterisation or other procedures involving instruments inside the urethra can cause injury and subsequent scar formation.
Prolonged Catheter Use
Long-term catheterisation may irritate or injure urethral tissue and can contribute to narrowing of the urethral opening.
Trauma or Injury
Injury to the penis or urethral opening may heal with scar tissue and result in narrowing.
Previous Circumcision
More common in circumcised children; in adults, it is usually linked to previous procedures, injury or inflammation.

How We Diagnose Meatal Stenosis
Accurate evaluation helps determine whether narrowing is limited to the urethral opening or extends deeper into the distal urethra, which can change the treatment approach.
- Medical & Surgical History – reviews urinary symptoms, previous circumcision, urethral surgery, catheterisation or instrumentation.
- Physical Examination – directly evaluates the size, shape and appearance of the urethral opening.
- Urinary Stream Assessment – checks whether the stream is weak, narrow, deflected or spraying.
- Uroflowmetry – measures the strength and pattern of urine flow.
- Post-Void Residual Test – may assess whether urine remains in the bladder after urination.
- Urethral Calibration – may help assess the degree of narrowing in selected patients.
- Cystoscopy – may be advised when a more extensive urethral stricture is suspected.
- Urethral Imaging – additional imaging may be considered when narrowing extends beyond the meatus or previous reconstruction has been performed.
