Microsurgical Spermatic Cord Denervation (MSCD or MDSC) is a specialized surgical procedure used in carefully selected men suffering from chronic testicular or scrotal pain that has not improved adequately with conservative treatment.
During the procedure, the spermatic cord is examined under magnification and selected nerve-bearing tissues believed to transmit chronic pain signals are divided while important structures supplying the testicle are carefully preserved.
The aim is to reduce chronic pain while preserving the testicle rather than removing it. MDSC is considered a testicle-sparing surgical option for selected patients with chronic scrotal content pain.
Important: Not every patient with testicular pain requires surgery. A detailed evaluation is necessary to identify reversible causes, and response to a diagnostic spermatic cord block can help predict whether microsurgical denervation may be appropriate.

Chronic Testicular Pain
Persistent or recurrent pain involving one or both testicles that significantly affects daily activities or quality of life.
Chronic Scrotal Content Pain

- Medical & Pain History – duration, location, severity, triggers and nature of the pain.
- Physical Examination – assessment of the testicles, epididymis, spermatic cord, groin and surrounding structures.
- Scrotal Ultrasound – may be advised to identify abnormalities such as masses, cysts, varicocele or other structural causes.
- Urine Tests – may be performed when infection or urinary disease is suspected.
- Previous Surgery Review – particularly important after vasectomy, hernia surgery, scrotal surgery or other pelvic procedures.
- Assessment for Reversible Causes – including infection, varicocele, hernia, testicular mass and other identifiable conditions.
- Spermatic Cord Block – a local anaesthetic injection around the spermatic cord may temporarily reduce pain and can help assess whether denervation is likely to be beneficial.
